correct the situation. Two out of four parents reported no concerns. P2 reported one teacher is adamant about keeping the facility clean. P6 reported observing soiled clothing on top of cubbies and stated the facility could use some cleaning. On 7/28, 9/11 and 9/26/25, LPA toured the facility and did not observe the alleged cleaning issues. LPA observed the kitchen clean and sanitary with food and cookware organized. It was also alleged that staff do not ensure infants are provided with adequate care and supervision at all times, specifically, staff leave children unattended in the classrooms. The director denied the allegation and stated there’s a great group of staff working at the facility and she has never observed staff leaving children unattended. Two out of four parents voiced concerns and stated one staff focused on one child. Two out of four staff members voiced concerns about a staff member but did not provide specific details and stated that the staff member is no longer working at the facility. On 7/28, 9/11 and 9/26/25, LPA observed staff providing adequate supervision to infants in care. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. Exit interview conducted and report was reviewed with the director, Kristen Walworth. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
