Inspector’s narrative

What the inspector wrote

An unannounced case management inspection was conducted today at 1:15p.m. by Licensing Program Analyst (LPA) Nicolette Cunningham. LPA met with director Shelly Anstine to discuss an Unusual Incident reported to the Department on 12/05/25, regarding an incident in which child (C1) was left outside for approximately 12 minutes. The director was interviewed on 1/9/26 at 2:00pm and stated that on 12/05/25 at around 11:42am, a staff member (Staff 1) transitioned a group of children from the playground to the classroom. During this time, one child (Child 1) was left on the enclosed playground for approximately 12 minutes. The incident was captured on video surveillance. Director Shelly Anstine stated Child 1 did not appear injured, scared or upset. During today\342\200\231s inspection, LPA requested video footage of the incident and the director she will send by 1/16/26. LPA photographed the area Child 1 was left outside. Based on report received and interviews conducted the following deficiency is being cited on the LIC809-D: 101229(a)(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. LPA informed site supervisor Shelly Anstine that this report dated 1/9/26 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Nicolette Cunningham informed the site supervisor to provide a copy of this licensing report dated 1/9/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the site supervisor Shelly Anstine. Appeal Rights were provided.
