Facility-requested visit·August 25, 2022

An inspection was made to the facility by Licensing Program Analysts (LPA), Snow who met with Scotti Gold to increase capacity, add a play yard and add room # 36 (they are currently operating in room #20 only). The facility file was reviewed prior to this visit. This is a Title 5 funded program operated by Enterprise School District that will operate in room 20 & 36 on the Alta Mesa Elementary School grounds with a total capacity of 39. (Room 20 @ 24 capacity and Room 36 @ 15 capacity) The facility\342\200\231s operating hours are 8-11am and 1:30-2:30pm, Monday-Friday scholl holiday schedule followed. The facility was toured inside and outside and the floor and yard plan submitted by the licensee were verified the adequate measurements, required square footage required. The facility has been approved for the full capacity by the fire department. *The facility applied to add an additional room #36 & additional play yard and an additional 20 children. Due to there only being one toilet and one sink designated Scotti Gold has decided to reduce the capacity (added 15 instead of 20) for a new total capacity of 39. A provisional license shall be granted effective 8/29/22. The following is still needed: 1. The new Director shall submit the director change documents within 30 days (list provided) 2. The waivers will need to be requested & processed for shared play yards 3. Original Application with the updated capacity of 39
