What Happened
Unannounced case management inspection following unusual incident report. Child left unsupervised on playground with unqualified maintenance staff during transition. Type A citation issued for supervision violation.

- Incident occurred during transition on 5/23/24 at approximately 12:00pm
- Child was hiding behind shelves outside

101229(a)(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Based on interview and unsual incdent report received, the facility did not comply with the section cited above in 1 count, which poses an immediate health, safety, or personal rights risk to children in care.
