What the inspector wrote

An unannounced case management inspection was conducted today at 10:08am by Licensing Program Analyst (LPA), Sydney Sims. LPA met with facility representative Makyla Waugh. In response to an Unusual Incident Report received by the Department on 10/08/25, where child C1's finger was smashed in a fire truck door by adult W1 during a presentation from the Fire Department. The facility representative was interviewed on 10/22/25 at 11:08am and stated that on 10/7/25 around 11:00am Child C1 was sitting in the cab of a fire truck with the preschool class for the community helpers presentation and adult W1 closed the entry door to the fire truck and did not see that child C1's fingers were in the door jamb. Facility Representative Makyla Waugh stated that staff were in ratio with five staff were present and 22 children outside and that staff S1 and Makyla both observed the incident. One child C1 was interviewed on 10/22/25 and stated that W1 did smash C1's finger in the fire truck door when C1 was inside the fire truck. C1 stated that there was a couple staff present who helped C1 but that C1 could not remember which staff were present. One Staff (S1) was interviewed on 10/22/25 and stated that C1's finger was smashed in the fire truck door by W1 and that S1 observed the incident occur. S1 stated there was five staff present outside with the children but could not recall the exact amount of children present but stated that the facility was in ratio and that proper supervision was being provided. One Adult (W1) was interviewed on 10/22/25 and stated that W1 was inside the fire truck engine with the children and that when W1 went to close the fire truck door to prevent children from falling out that W1 accidentally shut C1's fingers in the door. W1 stated that there was at least 3 staff outside that W1 could see and that the staff were right next to the doors of the Fire truck on both sides providing supervision. During today\342\200\231s inspection, the facility was toured and LPA Sims observed 70 children in care. Based on interviews conducted and report received by the department child C1 did sustain an injury at the facility but it was determined that the injury could not have been prevented. Exit interview conducted and report was reviewed with the facility representative Makyla Waugh. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. There were no deficiencies cited during today\342\200\231s inspection.
