What the inspector wrote

An unannounced case management inspection was conducted today at 10:46am by Licensing Program Analyst (LPA), Bianca Mendez and Erica Laird. LPA met with licensee Jenna Mazzanti. In response to an Unusual Incident Report received by the Department on 3/11/25. Child (C1) was walking outside and tripped over a tricycle and hit the back of his top of his upper inner lip. The licensee was interviewed on 3/27/25 at 10:46am and stated that on 3/10/25 at 2:40pm, child (C1) tripped over a tricycle and was bleeding from their mouth, licensee notified the parent immediately when the incident occurred. Licensee stated that the child was picked up immediately and was taken to the emergency room. LPA Mendez interviewed parent (P1) on 3/25/25 and stated that they were immediately notified, they were aware that their child hit their mouth and had tripped over a tricycle and had a internal mouth bleed and was taken to the emergency room. P1 stated that C1 did not require any stitches and a scabbed form over their injury. P1 stated that they no had no concerns about supervision. During today’s inspection, the facility was toured. LPAs observed 11 children in care and 1 staff on site. Based on information and interviews conducted, it could not be determined that the incident could have been prevented. Exit interview conducted and report was reviewed with the licensee Jenna Mazzanti. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days.
