Four children (C1 – C4) were interviewed on 01/21/26 and had knowledge of the allegation C1 – C4 stated that there is glass on the play ground and that the children will bring the glass to the staff to dispose of. On 01/21/26 at 2:55pm the facility was toured, and the LPA observed a piece of glass on the playground where the children were playing. During the investigation LPA conducted interviews with staff and children and toured the play ground that supported the allegation. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. Exit interview conducted and report was reviewed with the Director Niki Bull. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
