What the inspector wrote

On 6/18/25 at 2:37pm Licensing Program Analyst LPAs Bianca Mendez and Sydney conducted an unannounced case management inspection during an unannounced complaint inspection, LPAs toured facility and observed play yards in the infant room. LPAs observed 6 play yards and 2 cribs in the infant room. When LPAs toured facility they observed 2 infants sleeping in play yards. LPAs interviewed facility representative Niki Bull and stated that they have 5 infants in their infant room using pack n plays. At 3:13pm LPAs toured facility and observed 10 children and observed 2 cribs and 6 pack n plays. LPAs Bianca Mendez and Sydney Sims informed facility representative Niki Bull that this report dated 6/18/25 documents One Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Bianca Mendez and Sydney Sims informed the licensee to provide a copy of this licensing report dated 6/18/25 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Appeal Rights and a Notice of Site Visit were given. The Notice of Site Visit must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. All licensing reports are public information and must be made available upon request for at least three years.
