Inspector’s narrative

What the inspector wrote

On 3/10/26 @ 3:49pm Licensing Program Analysts (LPAs) Erica Laird and Sydney Sims conducted an unannounced case management inspection and met with director Niki Bull. Facility director Niki Bull informed LPA Erica Laird the infant play yard is off limits for the winter season and will not be used again until it is cleaned. Niki stated infants are currently using the outside deck as the outdoor play area. LPA Laird measured the outside deck and determined it contained enough square footage for 1.5 infants. Based on current capacity of 16 infants, LPA Laird determined the outdoor space is not adequate to meet the outdoor space requirements of 75 sq ft per child. It was also determined the facility failed to notify licensing of the change to outdoor activity space as required. Based on observations and statements made by the director, the following deficiencies are being cited on the attached 809D: 101238.2(a)Outdoor Activity Space and 101212(c)Reporting Requirements Exit interview conducted and report was reviewed with facility representative Nicole Ramsey. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
