Inspector’s narrative

What the inspector wrote

Three witnesses were interviewed on 09/17/2025 and two of the three witnesses corroborated the information provided by the complainant. Two witnesses stated they observed older infant children that were unusually hungry and/or thirsty at the time of pickup where they would either eat or drink a larger than normal amount of food/water for them at that time. One witness also stated their child was provided with their breast milk insufficiently warmed, so the child did not drink it on a regular basis. On 07/31/2025 the facility was toured by LPA Erica Laird, and the LPA observed staff maintains a feeding chart for infants updated daily and a binder for infant needs and services plans. LPA also observed infant formula stored in the cupboard, and breast milk name-labeled and dated stored in the refrigerator. LPA also observed one staff feeding an infant at the time of the tour. During the investigation, staff and witness interviews revealed that older infants are not provided free access to drinking water inside or outside, even during times of triple-digit temperatures, and at least three children displayed an unusual hunger or thirst at pickup time to the point they would eat/drink a larger than normal amount for them at the time of the day Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. LPA Cunningham informed Director O’Neal that this report dated 9/19/25 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Cunningham informed the director to provide a copy of this licensing report dated 9/19/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the Director, Lisa O’Neal. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
