What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

REPORTING REQUIREMENTS
The director shall acknowledge reading the Reporting Requirements: Each licensee or applicant shall furnish to the Department reports as required by the Department including, but not limited to, the following: Any unusual incident or child absence that threatens the physical or emotional health or safety of any child shall be reported within 24 hours and in writing within 7 days. This requirement is not met as evidenced by the director admission and lack of reports for at least 2 reportable incidents. Which poses/posed a potential Health and Safety risk to children in care.

To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidenced by: based on witness statements, Staff 1 did not comply with the section cited above, which poses an immediate health, safety, and personal rights risk to children in care.

The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature. Based on interviews, Staff 1 did not comply with the section cited above with two out of ten children which poses an immediate health, safety or personal rights risk to children in care.

RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION
The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time. Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews, Staff 1 was observed focusing on their cell phone instead of visually supervising children. This poses an immediate health, safety or personal rights risk to persons in care.
