What the inspector wrote

On 6/29/22 at 9:00 am, Licensing Program Analyst (LPA), N. Cunningham conducted a case management visit and met with the Director, Lisa O'Neal. The purpose of the case management was in response to an unusual incident report that was made on 6/23/23. It was reported that a child (Child 1) had an accident in the bathroom and staff did not immediately assist the child. It was also reported that another child (Child 2) was close to Child 1 and ended up getting feces on their arm. Staff cleaned up both children after the incident. Interviews corroborated that staff did not assist children with toileting as needed. This presents and immediate risk to children in care. Notice of Site Visit shall be posted for 30 days from today's visit. The following violation(s) of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided. Reports citing Type A violations are to be provided to parents/guardians of children currently in enrolled and to parents/guardians of children newly enrolled at the facility during the next 12 months. Parents/guardians must sign Form LIC9224 to be kept in each child's file.
