What the inspector wrote

An unannounced case management inspection was conducted today at 11:00 AM by Licensing Program Analyst (LPA), Noah Wheeler. LPA met with facility director (DIR) Lisa O’Neal. In response to an Unusual Incident Report received by the Department on 07/10/2025. A child (C1) fell from play structure slide onto pea gravel causing C1 to hit his forehead above right eye. The facility director was interviewed on 07/16/2025 and stated that on 07/10/2025 at 10:5 AM, C1 was on the playground outside on the green climbing play structure with slides and a tunnel and rock wall. DIR stated he wasn't supposed to be on that climbing structure due to his younger age, and the staff was trying to get him off it. DIR stated the staff member (S1), who was supervising the children, was trying to catch him as he was coming down, and the child jumped to sit down on the slide and ended up tumbling off the side of the slide and hit his forehead above his right eye in the pea gravel. DIR stated the Director (S2) from Bright Futures 1 contacted parents immediately after incident occurred and during pick up DIR explained to the father of C1 the incident and where it occurred. Continued on 809-C DIR stated that after nap time, she called the parents again informing them that C1 wasn't acting normal, "emotional", and wouldn't eat snack. DIR stated the reason for the children not being able to use part of the structure is because they go outside by classrooms, the "Two's room" where children who are transitioning from infant to Pre-K and are a mix of 2-3 year olds , can use the structures swings but not go up into the structure which is about 5'5" from the ground. DIR stated that yesterday, 07/15/2025, the parent informed her that they took C1 to get looked at and was found to have a concussion. DIR stated that parents pulled child from care on 07/15/2025. One staff (S1) was interviewed on 07/16/2025 and stated C1 climbed up structure and was asked to come back down, C1 jumped onto their bottom on slide when sitting down which caused them to “tumble” down slide, C1 fell off the side of the slide and had a “road rash” mark on their face above the right eye. S1 stated parents were contacted immediately after incident and when parents were picking up C1, DIR showed parents where the incident happened. During today’s inspection, the facility was toured, LPA photographed play structure and conducted interviews. LPA observed proper supervision and care. There were no deficiencies cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee. Appeal rights were provided.
