What the inspector wrote

On 6/29/2023 at 3:30pm Licensing Program Analyst (LPA) N. Cunningham conducted an unannounced case management inspection and met with licensee, Alesha Brown. Witness 3 reported on 6/1/2023 the licensee left a child (child 1) with another adult (witness 4) and left the facility to attend a graduation ceremony. Witness 4 confirmed they provided care for child 1 until the child’s parent arrived which was approximately 15 minutes. On June 10, 2023, Witness 4 obtained a clearance and was associated to the facility license. The following deficiency is being cited: Care was provided by an adult who does not have a criminal record clearance with the Department (see LIC 809D): LPA Cunningham informed licensee to provide a copy of this licensing report dated 5/1/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the licensee.
