What the inspector wrote

A prelicensing inspection was conducted today by Licensing Program Analyst (LPA), J. Snow. LPA met with facility representative Jeanette Martinez. Applicant is requesting to be licensed for # 32 children, including 8 infant, 12 toddlers & 12 preschool children. Capacity was adjusted to # during the inspection due to bathrooms, sq footage. Updated capacity: # 26 children as follows: 10 preschool, age 2-6 years in room #2 & dining area & 10 Toddlers, age 18-36 months in room #1, & 6 infants, age 0-2 years in room #3. Facility operates in entire building. Hours of operation will be Monday -Friday, 6:30am to 5:30pm. The indoor and outdoor activity spaces were toured, and the facility sketch was verified. The following areas will be off limits to children: the kitchen & staff room. These areas have been made inaccessible by means of door locks. There is an infant nap room, preschool and toddlers will nap on mats in their classrooms. The isolation area for sick children will be located in the office. The changing table is located in the bathroom. The outdoor play area is fully fenced. There are no climbing structures. There is no pool, spa, pond, fountain, or any other body of water on the premises. The poisons will be locked in a kitchen cabinet. The stairs are gated at the top and a second gate will be installed at the bottom leading to the play yard. gate. There is safe and age-appropriate furniture, toys, and play equipment available for children. see 809c A capacity worksheet was completed during the visit. The inside, room square footage measured 956 square feet. Children have access to at least 1 toilet, 1 potty chair & 2 sinks. The outside, yard square footage measured 1020 square feet. Based on above, which was not sufficient requiring a reduction in the requested capacity during todays inspection & a requiring a yard waiver. The following items need to be completed within 30 days and prior to the granting the license: 1. Waiver approval- send letter request 2. Updated fire clearance 3. Install a gate at the bottom of the stairs leading to the play yard. \342\200\223 send photo 4. Install a lock for poisons. \342\200\223 send photo 5. Purchase 2 cribs \342\200\223 send photo 6. The infant sleep wall needs to be 4 feet- send photo 7. Update admission agreement a. Toddler permission; add a signature line for parents to grant permission to move in/out of the toddler program b. (page5) add \342\200\230without notice\342\200\231 under termination conditions. Criminal Record Clearance - Child Care Centers: Jeanette Martinez was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. http://www.ada.gov/childqanda.htm see 809c APPLICANT WILL WAIT UNTIL FACILITY OPENS TO DETERMINE IMS NEEDS: Incidental Medical Services (IMS) policy was discussed. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
