What the inspector wrote
A prelicensing inspection was conducted today by Licensing Program Analyst (LPA), J. Snow. LPA met with facility representative Brandy Groves for a change of ownership for 455402531
MILLVILLE PRESCHOOL. Applicant is requesting to be licensed for # 24 preschoolers children are 2-5 in room # 2. Hours of operation will be Monday -Friday, 7:45am to 4:15pmpm. The facility is located on Millville Elementary school grounds.
The indoor and outdoor activity spaces were toured, and the facility sketch was verified. The following areas will be off limits to children: the kitchen. These areas have been made inaccessible by means of a lock. Children will nap on cots in the classroom. The isolation area for sick children will be located in the office.
The outdoor play area is fully fenced. There is a play structure with slide for children to play on and adequate cushioning underneath. There is no pool, spa, pond, fountain, or any other body of water on the premises. The poisons are locked under the sink. There is safe and age appropriate furniture, toys, and play equipment available for children.
A capacity worksheet was completed during the visit. The inside, room square footage measured 1748. Children have access to at least 2 toilets & sinks. The outside, yard square footage measured 2055.
Based on above, sufficient for the requested capacity inside & outside.
The following items need to be completed within 30 days and prior to the granting the license:
1. Waiver approval for shared bathrooms.
2. Waiver approval for shared yard.
LEAD TESTING-
For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).
If the facility has had a lead test completed within the last five years from the pre-licensing visit date and is using the same outlets, you may submit a waiver request from the lead testing requirement to the Regional Office.
Criminal Record Clearance- CCC
Applicant, Licensee, or Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
APPLICANT WILL WAIT UNTIL FACILITY OPENS TO DETERMINE IMS NEEDS

Review of records to be maintained-
CCC LPA reviewed with the applicant, licensee, or facility representative the LIC 311A, Records to be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant.
Safe Sleep- CCC
LPA discussed the safe sleep regulations with the facility representative. and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
MyChildCarePlan.org- CCC
The Applicant or Licensee was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Notice of Site Visit (for licensed facilities only)- CCC and FCCH A notice of site visit was given to applicant, licensee, or facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
