What Happened
Two children (C1, C2) left unattended on playground for approximately 5 minutes during staff transition to extended care program when staff failed to conduct required head count and roll call procedures.
Observations
- Children left alone on playground for at least 5 minutes
- Staff did not follow head count/roll call procedure during transition
- Children found by parents at side of large wooden shed
- Video footage confirmed children unattended
