What the inspector wrote

Licensing Program Analyst (LPA), Melchisedeck Augustin conducted a Plan of Correction (POC) visit to verify the facility's teacher-child ratio and follow up to a type B citation the facility received on 05/17/23 due to water lead testing results received from the California State Water Resource Control Board which showed lead exceedance. As a result of a substantiated complaint investigation which confirmed the classroom operated out of ratio. On 06/30/23, the facility submitted a written plan which detailed that staff received information on the ratio requirements and moving forward, staff are to notify management via walke talkies or telephone if the classroom is one child away from reaching the teacher-child ratio. Furthermore, staff would request for families to wait until additional staff arrived, and staff would not accept additional children in the classroom to prevent further violation(s) of ratio requirements, and if multiple staff called off, there were two Center Directors on standby to provide support. During today's visit, LPA observed one teacher supervising 25 children napping in the Discover Preschool (DP) classroom, one teacher supervising 15 children napping in the Preschool class, and an Aide supervising 17 children napping in the Pre-K class. Based on LPA’s observation, the DP class did not comply with the requirements of the teacher-child ratio which requires the facility to comply with California Code of Regulations, 101230(c) which mandates a teacher-child ratio of one teacher supervising 24 children napping. The test results from the California State Water Resource Control Board showed that the following faucet tested above the allowable level (5.5 ppb) of lead in the water: Sample site, "A", is a sink located in the hallway outside the "Infant classroom", had a reading of 19.8 ppb and Sample site, “D”, is a hand washing station that is located in the “kitchen” had a reading of 6.3 ppb, and Sample site, “G”, is located in a preschool classroom; had a reading of 12.0 ppb. All other sources of water that were tested had a reading below the allowable level of 5.5ppb. (Continue to LIC 809-C) The sink in the hallway outside the “Infant classroom” was removed and according to the Center Director, a plumber worked on, changed, flushed, retested for lead on 08/04/23, and remediated the plumbing for sample sites, “D” & G. Exit interview conducted and report was reviewed with the Center Director, Donje Fields. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. The following violation(s) of the California Code of Regulations, Title 22; Division 12, Chapter 1, Article 06 observed during today’s visit. See LIC 809-D. Appeal Rights were provided.
