What the inspector wrote

On 12/04/2024 at 2:28pm an unannounced case management deficiencies visit was conducted. Licensing Program Analyst (LPA), Cindy Castro was greeted by facility Cook, which informed LPA that Center Director (CD), Justice Willis was at a meeting. Facility Cook provided LPA the names of the two Lead Teachers left in charge. At 2:32pm while conducting census of facility classrooms. LPA observed Teacher (T1) in Discovery Preschool left alone with 20 children during naptime hours, 4 children were awake, 1 child was laying in napping cot awake, 1 child was walking in the classroom and using the bathroom and 2 children were playing with toys. T1 stated that Lead Teacher (T2) was out on her lunch break and would not be back for another 20-25minutes. LPA then requested to speak with the other Lead Teacher (T3) and was told by Teacher (T4) that T3 had left the facility for the day. T1 contacted T2 by text to return from her lunch, at 2:48pm T2 arrived at the facility Discovery Preschool Classroom to maintain ratio. At 2:51pm LPA phone called CD who told LPA that she was an hour away from the center but would be returning. LPA informed CD of Activities 101230 (C), Title 22 Regulation for a Child Care Center. Center Director arrived at the facility at 4:14pm. The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided and discussed. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted, and report was reviewed with Center Director, Justice Willis.

    101230 Activities (c) A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center. This Requirement was not met as evidence by: Based on observations by LPA at 2:32pm while conducting a census of facilty. One Teacher was left alone with 20 children during naptime with 4 children awake,which poses/posed a potential health, safety or personal rights risk to persons in care.
