What the inspector wrote

Licensing Program Analyst (LPA) Cindy Castro conducted an unannounced Plan of Correction (POC) visit and met new Center Director, Nikita Wallace to follow up on prior deficiencies issued on 10/30/25 for staff files and child files. LPA requested to review 4 staff files to verify proof of negative tuberculosis testing, up to date certificate of Mandated Reporting training and proof of immunization's against Pertussis and Measles. One child file was requested, to verify C1 immunization's were up to date.Center Director reported that S3 is an on-call staff and has not been present in the center and that an email will be sent to inform staff that upon returning to work at facility an updated Mandated Reporter Certificate and proof of immunization for TDAP and MMR is required. Center Director reported that S5 was out on personal leave and has a medical appointment scheduled on 11/21/25 to complete form LIC503 and TB test. Plan of corrections were cleared and copies of Cleared Plan of Correction Letter were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Center Director, Nikita Wallace.
