Inspector’s narrative

What the inspector wrote

A case management visit to the facility was made by Licensing Program Analysts (LPA) Y. Yang and D. Morrison. During today's visit, the LPAs met with the facility's new center director, Nikita Wallace who stated that she started as the center director in November 2025. During this case management visit, the LPAs provided the director with technical assistance and information regarding the change of director procedure, regulations, and documents to be submitted to the Department. The director provided the LPAs with a copy of her director's packet to be brought back to the regional office for processing. The LPAs reviewed the packet and observed that the only document missing was the LIC 200A, which the director stated would be submitted at a later time by an individual within the organization with the authority to sign the document. Additionally, the LPAs reminded the director that in situations when the child care center director is absent from the center, arrangements shall be made for a fully qualified teacher as specified in Section 101216.1(c) to act as substitute. This substitute child care center director shall be aware of center operations, including total enrollment; shall be trained in program operation; and shall be designated as an authorized person to correct operational deficiencies that constitute immediate threats to children's health and safety. Additionally, if the child care center director is absent for more than 30 consecutive calendar days, the substitute director shall meet the qualifications of a director. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the center director, Nikita Wallace. There were no Title 22 deficiencies cited during today's inspection.
