What the inspector wrote

Licensing Program Analyst (LPA) Selena Mariani made a visit to the facility for the purpose of delivering complaint investigation findings. During the preschool license # 483001838 complaint investigation (Control Number 01-CC-20251209134223), additional deficiencies were identified and a case management report was added to the visit. LPA met with Assistant Director (AD), Nakia Orlando. During a complaint investigation, LPA conducted interviews with center director (CD) Nikita Wallace, 10 adults (current staff, former staff & parents: A1-A7, A9 & A10-A11) which revealed by admission that adult 5 (A5) supervised an infant child (C4) in the preschool classrooms. Additionally, A1 and A3 corroborated that A5 would supervise C4 in the preschool classrooms. The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC809-D. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Assistant Director (AD), Nakia Orlando.
