What the inspector wrote

An unannounced case management visit was made to the facility by Licensing Program Analysts (LPAs) Selena Mariani and Jessica Gaumann to follow up from case management visit on 10/20/25 regarding a self-reported Unusual Incident Reports (UIRs) which was reported to CCL on 10/13/25. LPAs met with Licensee (LS) Rose Thomas. During today's case management visit LPAs conducted staff file review. No deficiencies were cited as a result of this case management visit. This report was read and reviewed with Licensee Rose Thomas. Notice of site visit shall be posted for 30 days.
