What the inspector wrote

Director Mel McMillen was interviewed at 9:20am when she arrived and stated that current plumbing issues began just before Christmas, and bathrooms in the Sharks room and staff bathrooms are inoperable as a result. Children use the hall bathroom (children's bathroom) and staff uses the deli next door. She stated that the owners are in touch with several plumbers and the holidays has delayed the repairs. She denied knowledge of mold/mildew. Five staff were interviewed on 1/10/25 and all described sewage overflowing in the Sharks room the second week of November or thereabouts, and the subsequent plumbing work which has been disruptive and has not fixed the issues. All interviewed indicated that the plumbing problems have caused either a sewage or mildew/mold odor intermittently; strongest when the plumbers were actively working. During today’s inspection, the facility was toured and facility roster obtained. LPA smelled a strong bleach/cleaner odor throughout the facility, observed one staff toilet was detached from the sewer pipe and inoperable, and that the Sharks' bathroom was inoperable. Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
