What the inspector wrote

An unannounced case management inspection was conducted 10/7/25 at 11:00 am by Licensing Program Analysts (LPA), Emily Curiel and Tammy Dutra. LPAs met with director Mel McMillin in response to an Unusual Incident Report received by the Department on 7/24/25: Child (C1) disclosed that another child (C2) inappropriately touched them. The director Mel McMillin was interviewed on 9/30/25 at 11:00 am and stated that on 7/21/25 there was one staff supervising no more than 14 children. McMillin stated that there is constant monitoring of children by staff. McMillin stated no staff witnessed C1 being inappropriately touched by C2. McMillin stated that they have interviewed C2 who was "adamant" that they did not touch C1. McMillin stated that they were aware of another incident regarding four children (C1-C4) that was addressed immediately, and the parents were made aware of the situation. McMillin stated that there has not been another incident of a child touching another child inappropriately. One child (C1), one parent (P1) and 4 staff (S1-S4) were interviewed on 7/23/25, 7/24/25, 7/29/25, 8/6/25, 8/10/25 and 10/3/25. Staff (S4) stated that since the alleged accident staff continue to provide constant supervision to students in the classroom. S4 stated that children are being taught to have body boundaries and children are reminded not to go in areas with less visibility like under the playground in the outdoor play yard. Attendance records for staff and children were reviewed for 7/21/25 and 7/22/25. The facility was operating within ratio requirements on both 7/21/25 and 7/22/25. Based on interviews and record review, it could not be determined that a lack of supervision occurred. There are no deficiencies cited for this case management incident report. Exit interview conducted and report was reviewed with the director Mel McMillin. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
