What the inspector wrote

On 12/18/25, an unannounced case management inspection was conducted by Licensing Program Analyst (LPA) Emily Curiel. LPA met with Director Lisa Erlandsson-Ward in response to an Unusual Incident Report received by the Department on 10/31/25 regarding two children (C1 and C2) that were left behind in classroom. The director, Lisa Errlandson-Ward was interviewed on 11/3/25 and stated that on 10/31/25 the teachers and students were preparing to leave classroom to do the annual autumn parade. The students held on to a rope and teachers did a headcount prior to leaving the classroom. Two parents (P1 and P2) noticed their children were not present in the parade and alerted the staff. One member of staff (S1) ran to the classroom where they found C1 and C2 happily playing with lights that were on C1's costume. Errlandson-Ward estimates that C1 and C2 were left in the classroom without supervision for 2-3 minutes. On 12/18/25 Errlandson-Ward stated since the10/31/25 incident, plans been made to prevent children from being left without supervision for next year's parade and for everyday transitions. Four Staff (S1 - S4) were interviewed on 11/12/25 and stated that C1 and C2 were left behind in the classroom for approximately 3-5 minutes. S4 stated that when they found C1 and C2 and that the children were sitting on the floor playing. S1 stated they walked in and saw C1 and C2 looking at the lights on the costume. No staff stated that the C1 or C2 appeared distressed when they were found in the classroom. Based on information reported and interviews conducted the following deficiency is being cited on the LIC809-D: 101229(a)(1) No child(ren) shall be left without the supervision of a teacher at any time. Exit interview conducted and report was reviewed with Lisa Erlandsson-Ward. Notice of site visit was provided and must remain posted for 30 days.
