What Happened

Complaint investigation follow-up on nap time supervision. Two allegations regarding blanket placement over child's head: one substantiated, one unsubstantiated based on police report and LPA video review.

Observations

  • Allegation 1: SUBSTANTIATED - Police report and video showed staff S1 place blanket over child C1's head, child removes blanket, staff replaces blanket over child's head
  • Allegation 2: UNSUBSTANTIATED - While video showed S1 laying child C1 back down on cot and placing blanket and repositioning child, preponderance of evidence does not support violation claim
  • Police report dated 10/10/22 documented video review from 08/18/22 showing blanket placement behavior
  • Citation issued for substantiated allegation under Title 22, Division 12, Chapter 1, section 101223(a)(3)
