What Happened
Annual inspection of family child care home with two deficiencies: missing immunization documentation for all children and missing sleep logs for infants.
Observations
- DEFICIENCY: All three children missing blue CDPH 286 immunization forms
- DEFICIENCY: Infants in care missing required sleep logs (Type B)

Based on interview and record review, Licensee was not documenting sleep log for all infants in care. Licensee was keeping a daily log for C1, but was not documenting date, infant's name or time of each 15 minute check. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
