What the inspector wrote

On 11/14/2024 Licensing Program Analysts (LPAs) Elpidia Hernandez Torres and Mindy Mohr were at the facility to deliver complaint findings. Upon arrival LPAs observed the facility had two banners located outside, one banner on the front of the building, and the other banner on the front lawn advertising with the intent of attracting clientele. These banners do not contain the facility number as required. It was also revealed the facility has a website that also does not contain the facility number as required. The following violations of California Code of Regulations, Title 22; Division 12, were observed during today\342\200\231s visit. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights provided. Exit interview conducted and report was reviewed with Director, Sabrina Oldani.

  • INFANT CARE TEACHER QUALIFICATIONS AND DUTIES

    101416.2 Infant Care Teacher Qualifications and Duties. Prior to employment, an infant care teacher shall have completed, with passing grades, at least three postsecondary semesters or equivalent quarter units in early childhood education or child development, and three postsecondary semester or equivalent quarter units related to the care of infants, at an accredited or approved college or university. This was not met as evidence by... Based on interviews conducted and records reviewed aides were being used in the capcity of a teacher being left alone with infants. This poses a potential health and safety risk to infants in care.

    101416.5 Staff-Infant Ratio(b) There shall be a ratio of one teacher for every four infants in attendance. This was not met as evidence by... based on interviews condcuted, one staff was observed providing care to 5, 6, or 7 infants at one time with one occasion, one aide was providing care to 3 infants. This poses a poetential health and safty risk to infants in care.

    Based on record review, a disaster drill was not condcuted within the last 6 months. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

  • IMMUNIZATIONS

    Based on observation, record review, only one infant out of seven had immunizations on file. All infants were also missing LIC 701 physicans report. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on on reviewing 7 infant records, two infants did not have a record at all and all infants's files were incomplete. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION FOR INFANTS

    Based on observation, interview, and record review, the infant staff are not documenting the infant safe sleep logs in the moment the infants are being checked on. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on observation, the infant changing table that is being used is across the napping area not within arms reach. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

    Based on interview and record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

    Licensees shall reveal each childcare center license number in all advertisements in accordance with Health and Safety Code Section 1596.861.This requirement is not met as evidenced by: Based on LPA's observation of two banners and website the facility used to advertise with the intent of attracting client(s), however the banners did not contain the facility number.
