What the inspector wrote

An unannounced case management visit was made to the facility by Licensing Program Analysts (LPAs) Y. Yang and M. Mohr in response to a self reported incident involving child C1, staff S1, and staff S2 that occurred on 02/22/23. During today’s visit, the LPAs met with the center director, Alex Pepper to discuss the incident. It was reported by center director Pepper that on 02/22/23, while on the center's main preschool playground, child C1, a preschool age child, separated from their classmates and returned inside the center and walked to the “Toddler B classroom” without the supervision, knowledge, or escort of a staff member. The center director stated that C1 and another child were using a bathroom in the "Preschool Classroom" which is adjacent to the main play yard and has its own exterior door leading to the main play yard. The center director stated that S2 was positioned at the exterior door supervising the two children using the bathroom inside the classroom. The center director stated that once C1 was done using the bathroom, C1 returned to the main play yard and immediately opened the center's hallway door separating the center's hallway from the center's main play yard and went inside the hallway towards the Toddler B classroom. The center director stated that the other child was still using the bathroom at this time and being supervised by S2. The center director stated that C1's authorized representative was in the Toddler B classroom at the time. The center director stated that the Toddler B classroom is located down the hall from the door separating the center's hallway from the play yard. The center director reported that based on the “Child Supervision Record” (CSR) for the day, Staff S1 and S2 were supervising a total of 20 children at the time while on the play yard. The center director stated that C1 was in the center's hallway without staff supervision for approximately one or two minutes before being spotted by the center director. The center director reported that she then escorted C1 back to their group, on the play yard and notified C1's authorized representative. The center director stated that S2 was still standing at the doorway supervising the other child in the bathroom when C1 was returned to the group. The center director stated that S1 was on the play yard supervising a group of children on the play structure. This self reported incident was reported to CCLD as required per regulations. During today’s case management visit, no children were observed to be left without supervision. Based on available information, there is not a preponderance of evidence to suggest that the incident on 02/22/2023 occurred due to a lack of staff supervision or any other violation or regulations. This report was read and reviewed with the center director, Alex Pepper. Notice of site visit shall be posted for 30 days.
