Inspector’s narrative

What the inspector wrote

During the course of a complaint investigation, additional deficiencies were identified. On 4/13/2026, LPA met with Director Ana Alvarenga (D1). LPA observed 10 children being supervised by D1 and 2 staff. During LPA's file review, 5 of 5 children's files did not document injuries to the child's authorized representative. D1 stated that injury incidents are not documented in the children files but authorized representatives are notified verbally. S1 stated that the facility uses Brightwheel but not for reporting incidents to parents. The following violations of the California Code of Regulations, Title 22; Division 12: see LIC 809D. Appeal rights were provided. This report was reviewed and discussed with the Director, Ana Alvarenga. A Notice of Site Visit shall be posted for 30 days from today’s visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
