What the inspector wrote

On 04/13/2026, Licensing Program Analysts (LPA), Sebastian Phouthaovong made an unannounced Case Management visit in response to a self-reported, lack of supervision Unusual Incident Report (UIR). UIR was received by Community Care Licensing on 03/16/2026. The incident was regarding two children being unsupervised in the indoor area. LPA met with Site Supervisor, Michelle Martin and spoke with them about the purpose of today's visit. During today's inspection, LPA toured the facility inside and out and observed 18 children being supervised by 5 staff members. LPA conducted interviews with the Site Supervisor and two staff members. Site Supervisor stated when Staff 2 (S2) was transferring a daycare child to the outdoor area, two daycare children remained indoors with Staff 1 (S1). However, S1 was not aware that the two children were indoors with them. S1 also confirmed that they were not aware the two daycare children were indoors with them and that S1 was in another section of the classroom from the two children, leaving the children unsupervised. In addition, the incident was addressed and Site Supervisor stated the facility supervision procedures will be reviewed to staff. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Site Supervisor, Michelle Martin The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809 D. Appeal rights were given.
