A Case Management inspection was conducted at the facility by Licensing Program Analyst (LPA) Y. Yang in response to an Unusual Incident Report (UIR) received on December 3, 2025, concerning a child who sustained a head injury while in care. During the inspection, the LPA met with the center director, Lisa Steindorf, to discuss the incident and gather additional information. The director stated that on the morning of December 1, 2025, child C1 was playing in the center\342\200\231s play yard and was seated in the window opening of a wooden playhouse. The LPA observed that the window frame of the playhouse was approximately one foot off the ground. According to the director, C1 lost their balance, fell backward, and struck their head on a tire positioned on the ground. The director reported that a staff member, who was positioned next to the playhouse, observed the incident, assessed C1 for injury, brought the child indoors, and provided an ice pack. The director noted that prior to the fall, a staff member had reminded C1 that sitting in the playhouse window was unsafe and attempted to redirect the child. The director reported that C1\342\200\231s authorized representative was contacted. It was mutually agreed to keep C1 at the center for the remainder of the day and to monitor for additional symptoms. The director reported that C1 resumed play and remained at the center for their regular hours. Later that evening, the authorized representative informed the director that C1 began exhibiting symptoms consistent with a concussion and was evaluated by a medical professional, who diagnosed a mild concussion. Following this incident, the director indicated that children are no longer permitted to sit in the playhouse window openings and will be immediately redirected to a safer area. During today's case management visit, the LPA reviewed with the director the facility's child supervision policies. The LPA inspected the outdoor play area and did not observe any hazardous items or conditions. The LPA also observed that there is also sufficient wood chip cushioning material in areas with equipment that requires fall protection cushioning. This incident was reported to CCLD and the child\342\200\231s authorized representative as required per regulations. Based on available information, it has been determined that there was no evidence of violations of regulations that occurred that lead to the child's injury. This report was read and reviewed with the center director, Lisa Steindorf. There were no Title 22 violations cited during today's case management visit. The Notice of Site Visit shall be posted for 30 days.
