What the inspector wrote

On 06/26/2025, Licensing Program Analysts (LPA), Selena Mariani made an unannounced Case Management visit in response to a self reported Unusual Incident Reports (UIR). Community Care Licensing received the written UIR on 06/20/2025. The incident was regarding a child who tripped and fall off low step and later was running bare foot in sandy yard while at the facility leading to injury. LPA met with Center Director (CD) Josephine Jacob to discuss the purpose of today's visit. During the inspection, LPA toured the facility inside and out, observed 13 children being supervised by 3 staff and conducted child's record review. CD stated the child has a known medical condition and was previously allowed to play in the yard bare foot. Due to the child's medical condition and injury, shoes will be required while playing in the play yard. CD has spoken to staff to ensure a safe environment for the child. Based on LPA's observation and documents obtained, there was not a preponderance of evidence to prove that a violation of personal right occurred. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Center Director (CD) Josephine Jacob. Appeal rights provided. There were no Title 22 deficiencies cited during today's inspection.
