What the inspector wrote

An inspection was conducted at the facility by Licensing Program Analysts (LPAs), Jen Patel and Sebastian Phouthavong. LPAs met with Site Supervisor, Jamie Howlett. During the inspection the facility was toured inside and outside. 15 children were being supervised by 4 staff and operating within the licensed capacity and ratio requirements. During the course of an investigation on 10/23/2025, LPAs, Patel and Rosales observed two children that were no properly supervised in the corner of the play yard. Also, from one staff interview conducted on 10/23/2025, they stated that there was an incident where a child was injured however was not observed by them. Site Supervisor reviewed the requirements for Supervision and stated that three staff are always present with the children. When outdoors, two teachers would locate themselves at the end of the yard, while one staff would walk around as the children played. Additionally, SS and two staff stated that the facility ratio is 1 staff member to 8 daycare children. Furthermore, SS reviewed the reporting requirements and stated if the injury is a large open wound, profusely bleeding, or a major head injury, they will report it to CCL. SS was reminded to notify CCL within 24 hours of the injury occurring and complete the written report within 7 days. SS was provided regulations 101212 Reporting Requirements and 101226.3 Observation of the Child. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Site Supervisor, Jamie Howlett. No Title 22 deficiencies were cited during today's inspection.
