What the inspector wrote

An unannounced case management visit was made to the facility today by Licensing Program Analysts (LPAs) Robert Maciel and Jamie Clark for the purpose of following up an unusual incident report (UIR) submitted to the department on 12/30/25 regarding a child (C1) who broke his leg and met with Director Donya Olocco Parham (D1). According the UIR, a parent of C1 notified the facility on 12/22/25 that C1 had broke their leg and that it happened at the facility. Staff present at the facility did not observe any incident in which C1 was injured or any symptoms of injury. During today's visit, LPAs interviewed D1, staff (S1 and S2), C1, and the parent of C1, P1. During interviews with staff, D1, S1, and S2 stated that when C1 was present on 12/18/25 - 12/19/25, there were no signs that C1 was injured and C1 was acting and playing normally. Interview of C1 did not reveal the cause of the injury. During interview, P1 stated that on 12/18/25, C1 told P1 that they had hurt their leg at school but P1 did not observe any signs of injury and C1 was able to walk. On 12/19/25, C1 attended the school without incident and on 12/20/25, C1 complained again of leg pain which is what prompted P1 to seek medical treatment. No deficiencies were observed during today's visit. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days.
