Inspector’s narrative

What the inspector wrote

During the course of a complaint investigation, Licensing Program Analysts (LPAs), Leticia Rosales-Meza and Ivet Perez, conducted an unannounced Case Management visit, and met with Licensee (LS), Diana Cuevas, to deliver a citation. The Department’s Investigation Branch (IB) obtained law enforcement reports and records from other external agencies, which reported two minor victims were forensically interviewed, respectively confirming two minors’ personal rights were violated by another child (A3/MC3), between 2011–2013 & 2016-2018. A3/MC3 was also interviewed by PD and A3/MC3 admitted to sexually abusing a former daycare child. Furthermore, on 03/13/26, LS validated that MC3 was arrested, convicted, and served jail time for violation of a child’s personal rights. According to Department records, LS did not notify the Department of the incident and did not comply with reporting requirements. This report was discussed and reviewed with Diana Cuevas, and an Exit Interview was conducted. Notice of Site Visit shall be posted for at least 30 days. The following Title 22, California Code of Regulations is being cited on the attached LIC 809D. Appeal Rights were provided.

This requirement is not met as evidenced by: Based on an interview, Licensee did not notify and/or submit a UIR to CCL. This posed a potential health, safety and/or personal rights risk to children in care.
