Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Amy Strother made an unannounced case management visit to the facility as follow up on a visit made to the facility on 03/13/25, and met with facility representative, Center Director, Sydney Soos. LPA\342\200\231s visit to the facility on 03/13/25 was conducted in response to receiving an Unusual Incident Report (UIR) on 02/28/25, regarding a possible violation of children\342\200\231s personal rights occurring on 02/26/25. On 03/10/25, LPA received and reviewed a report from the Santa Rosa Police Department, documenting that on 02/26/25 a teacher at the facility, Staff 1 (S1) was believed to be under the influence of alcohol, placed under arrest and released to the Center Director. Based on LPA\342\200\231s interviews with staff, no staff suspected S1 was under the influence of alcohol on 02/26/25. The facility took immediate action, placing S1 on immediate suspension and then terminating S1\342\200\231s employment and removing S1 from the facility roster on 03/17/25. On 03/24/25 the facility submitted an updated LIC500 Personnel Report to the Department. Based on the information available at this time, there is not sufficient evidence to indicate that there was a violation of the regulations by the facility. No citations were issued during today's visit. Exit interview conducted and report was reviewed with facility representative, Sydney Soos. A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
