What the inspector wrote

A case management inspection was conducted by Licensing Program Analyst (LPA) Amy Strother for the purpose of confirmation of removal of an excluded individual. LPA met with Center Director, Sydney Soos. The Director stated that the individual is not present at the facility or working at the facility. The Director confirmed that she is aware that the individual is not permitted to be present at the facility at any time when children are in care. LPA toured the facility. Based on evidence obtained during today\342\200\231s visit, the LPA has verified the individual is not present, employed, or residing at the facility. LPA verified that the excluded individual has been removed from the criminal record clearance roster and has received an updated LIC500. The facility representative, Sydney Soos understands and is aware that an immediate $500 Civil Penalty will be assessed for having any adults work or live in the facility, without a background clearance. This report was reviewed and discussed with facility representative, Center Director, Sydney Soos. All Licensing reports are public information and must be made available upon request for at least three years. Notice of Site Visit shall be posted for 30 days from today's inspection. There were no Title 22 deficiencies cited during today's inspection. Verification of removal is complete.
