Inspector’s narrative

What the inspector wrote

An unannounced visit was made to the facility today by Licensing Program Analyst (LPA) Y. Yang. A review of staff records on 02/03/2026 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. There is currently one adult living in the home. During today’s inspection the home and grounds were toured. The licensee and two assistants were supervising nine children and operating within ratio. The facility’s operating hours are 07:30am to 05:30pm, Monday through Friday. The floor plan submitted by the licensee was reviewed and verified. The off-limits areas of the home were made inaccessible by door locks and/or child gates. The home was at a comfortable indoor temperature. The home's fireplace is latched and is not used during childcare hours. There were safe toys and equipment available for children. The licensee stated there is a working telephone in the home(the licensee's' cell phone). The licensee's pediatric CPR and First Aid certificates and CA mandated reporter training certificates expire in February 2026 and May 2026, respectively. Items which could pose a danger to children (such as detergents, cleaning compounds, medications, etc.) were observed to be stored out of the reach of children. The LPA observed a working smoke detector, carbon monoxide detector, and fire extinguisher, rated at least 2A10BC, in the home. The licensee stated that poisons, firearms, and ammunition are not stored on the premises. The home's backyard is fully fenced and currently off limits due to the presence of construction materials in the yard. The licensee shall notify the LPA once the items have been removed from the yard to make the yard on limits again. There were no bodies of water observed on the premises during today's inspection. Continued on LIC 809-C Five children's record were reviewed during the inspection. The LPA reviewed completed infant safe sleep plans and 15 minute nap check logs. The LPA reviewed three personnel files. Items were observed missing from both staff and children's records. The LPA provided the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as a resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. The Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Exit interview conducted and report was reviewed with the licensee, Sherine Zaki. There were no Title 22 regulations cited during today's inspection. During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.     Based on a personnel record review performed during the inspection visit, the licensee did not comply with the section cited above in 102369(b)(9). The LPA observed that staff S1 did not have proof of a current tuberculosis clearance in their personnel file. This poses/posed a potential health, safety or personal rights risk to persons in care.

  • IMMUNIZATIONS

    Based on a children's records review performed during the inspection, the licensee did not comply with the section cited above in 102418(g)(1). The LPA observed that children C1, C2, and C3 did not have completed PM 286 immunization logs in their file. This poses/posed a potential health, safety or personal rights risk to persons in care.
