What the inspector wrote

On February 03, 2026, a Case Management inspection was conducted at the facility by Licensing Program Analyst (LPA) Y. Yang in response to an Unusual Incident Report (UIR) involving a child injury. During the visit, the LPA met with the facility\342\200\231s co-licensee, Paige Schlager, to review the incident and gather additional information. P. Schlager stated that on 01/13/26, child C1 was playing in the facility\342\200\231s backyard on a swing. P. Schlager reported that while C1 was lying stomach facing down on the swing, the child swung forward and struck their head on a tree before P. Schlager could intervene. P. Schlager reported that an ice pack was applied immediately. P. Schlager reported that because C1 began exhibiting symptoms consistent with a concussion, emergency medical services were contacted. P. Schlager reported that co-licensee Jennifer Schlager supervised the other three children present while she attended to C1. P. Schlager reported that she rode in the ambulance with C1 so that C1 could be evaluated by a medical professional. P.Schlager reported that no additional medical treatment was required. During today\342\200\231s case management visit, the LPA reviewed the facility\342\200\231s child supervision policies with the licensee and provided best practice recommendations. The LPA also inspected the outdoor play area. P. Schlager stated that the original swing has been replaced with a different model to help prevent a similar incident in the future. Additionally, the new swing is now inaccessible unless a staff member is positioned next to it while it is in use. This incident was reported to Community Care Licensing and to the child\342\200\231s authorized representative as required by regulations. Based on available information, it has been determined that there was no evidence of violations of regulations that occurred that lead to the child's injury. This report was read and reviewed with the licensee, Paige Schlager. There were no Title 22 violations cited during today's case management visit. The Notice of Site Visit shall be posted for 30 days.
