What the inspector wrote

An unannounced case management visit was made to the facility by Licensing Program Analyst (LPA) Y. Yang in response to a self-reported, lack of supervision incident involving child C1 and staff S1 that occurred at the facility on 11/25/24. During today’s visit, the LPA met with the center director, Mackenzie "Kenzie" Miller to discuss the incident. It was originally reported by Center Director Miller that on 11/25/24 at approximately 10:50am, during a transition from one preschool classroom to another classroom across the hallway, preschool child C1 was left unattended in the classroom without staff supervision for approximately one minute. Center Director Miller reported that staff S1 was supervising a total of 10 children when this incident occurred. Center Director Miller reported that she was walking down the hallway and observed C1 alone in their classroom crying. Center Director Miller reported that she then brought the child to her office and sent a teacher to relieve staff S1 of her duties for the day. Staff S1 was interviewed during today’s visit and corroborated the director’s statements. Staff S1 stated that she believes that C1 was deliberately hiding in the classroom during this transition time. Center Director Miller reported that additional training was provided to S1 regarding “name to face transitions” using the facility’s internal training materials. Center Director Miller stated that C1’s authorized representative was notified of this incident the same day. This incident was also reported to Community Care Licensing as required per regulations. There were no Title 22 deficiencies cited during today’s inspection visit. A notice of site visit was given and must remain posted for 30 days. Technical assistance and best practice recommendations were provided by the LPA during today’s case management visit. This report was read and reviewed with the center director, Kenzie Miller
