    (c) In addition to the events specified in Health and Safety Code Sections 1597.467(b)(1)(A) through (b)(1)(C), the licensee shall report the following events to the Department: (1) Any suspected child abuse or neglect, as defined in Penal Code Section 11165.6, of any child in care, in addition to reporting requirements pursuant to Penal Code Section 11166.This requirement was not met as evidenced by: Based on LPA Strother’s interview with Licensee (L1) on 05/15/25, a child in care (C1) made claims of physical abuse to L1 on 05/13/25. L1 did not report the incident to CPS with a Suspected Child Abuse Report (SCAR) or report to Community Care Licensing Division (CCLD) as required, which posed a potential health, safety and/or personal rights risk to children in care.

    102417(e) When a child shows signs of illness he/she shall be separated from other children and the nature of the illness determined. If it is a communicable disease he/she shall be separated from other children until the infectious stage is over.This requirement was not met as evidenced by: Based on interviews conducted on 05/15/25 and 05/15/25, child C1 was excepted into care on 05/13/25 and in attendance between 7:00am and 3:48pm after vomiting during their arrival to the facility and comingled with 5 other children in care on 05/13/25, which posed a potential health and safety or personal rights risk to children in care.
