What the inspector wrote

Licensing Program Analyst (LPA) Amy Strother made an unannounced visit to the facility for the purpose of delivering complaint investigation findings, adding this Case Management report to the visit. LPA met with Licensee, Mayra Escobedo (L1). During the complaint investigation (Control Number 01-CC-20250514113554), additional deficiencies were identified. During an interview with L1 on 05/15/25, it was stated that a child (C1) arrived to the facility at 7:00am on 05/13/25 and vomited in the doorway upon arrival. An interview conducted on 05/15/25 with adult 1 (A1) corroborated that child (C1) was accepted into care after vomiting upon arrival on 05/13/25. A1 also reported during the interview that C1’s sibling stayed home from the facility on 05/08/25 due to vomiting. On 07/15/25 L1 stated that a total of 6 children were in care on 05/13/25, including C1 who was in attendance from 7:00am to 3:48pm. L1 additionally stated in the interview with LPA Strother on 05/15/25 that when C1 woke up from a nap on 05/13/25, L1 observed marks on C1’s chin, asking C1 what happened, stating that C1 said, “Mommy slapped me”, then said, “My mommy hit him”, when L1 asked C1 what happened to their siblings’ lip. L1 stated that she did not report C1’s disclosure to the required authorities, CPS and Community Care Licensing and did not discuss the marks observed with C1’s authorized representative on 05/13/25. During the complaint investigation, LPA received evidence that adult outside of the facility reported the marks on C1’s chin to local law enforcement, Child Protective Services and Community Care Licensing. The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC809-D. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Mayra Escobedo.

L1 did not report the incident to CPS with a Suspected Child Abuse Report (SCAR) or report to Community Care Licensing Division (CCLD) as required, which posed a potential health, safety and/or personal rights risk to children in care.

If it is a communicable disease he/she shall be separated from other children until the infectious stage is over.This requirement was not met as evidenced by: Based on interviews conducted on 05/15/25 and 05/15/25, child C1 was excepted into care on 05/13/25 and in attendance between 7:00am and 3:48pm after vomiting during their arrival to the facility and comingled with 5 other children in care on 05/13/25, which posed a potential health and safety or personal rights risk to children in care.
