What the inspector wrote

A case management inspection was conducted at the facility by Licensing Program Analyst (LPA) Yang in response to an Unusual Incident Report (UIR) received on May 20, 2026 regarding a school age child (child C1) who attempted to leave the program area during care. During the inspection, the LPA met with Site Director Chris Sachen to discuss the incident and obtain additional details. This center is located on the premises of Penngrove Elementary School. The director reported that on May 18, 2026, at approximately 8:00 a.m., child C1 was walking toward the upper playground area after speaking with him. According to the director, C1 was walking several feet ahead when the child turned around and asked where their mother was. After being informed that their mother had dropped them off and left campus, C1 ran from the program area toward the lower parking lot. The Director stated that C1 was unresponsive to verbal redirection and co-regulation attempts. He reported that C1 attempted to leave the parking lot area on multiple occasions; however, he positioned himself at the exits and physically blocked the child's path to prevent C1 from entering the parking lot or leaving campus. The director further stated that he remained with C1 on the sidewalk adjacent to the pickup area and continued utilizing regulation techniques to assist the child in calming. According to the director, the school principal responded to the area and provided additional support. Both adults offered C1 choices and encouragement to return to campus. After approximately 20 minutes, C1 voluntarily walked with staff to the elementary school's front office, where care and supervision of the child were transferred to school personnel. (Continued on LIC 809-C) The director reported that both of C1's authorized representatives were notified of the incident via text message shortly after the event. He further stated that the acting Champions Area Manager and the school district's Expanded Learning Opportunities Program (ELOP) Coordinator were informed of the incident that same morning. During the inspection, the LPA reviewed the facility's supervision and behavior guidance procedures with the director. The facility reported that a three-incident progressive guidance action plan has been implemented for C1 and that staff are completing daily Antecedent-Behavior-Consequence (ABC) charts to document behaviors and identify triggers. Additionally, the facility has collaborated with C1's authorized representatives to develop a modified morning drop-off plan that includes increased supervision, parent involvement, and additional support during transitions into care. Based on information obtained during the inspection, facility staff responded immediately to maintain supervision of C1 and prevent the child from leaving the premises. The facility has implemented corrective measures and individualized support strategies designed to reduce the likelihood of future elopement behaviors and assist the child with successful transitions into care. Based on the available information, no deficiencies were observed or cited during today's inspection. An exit interview was conducted, and this report was read and discussed with Site Director Chris Sachen. A notice of site visit was provided and must remain posted for 30 days.
