What the inspector wrote

A Case Management inspection was conducted by Licensing Program Analyst (LPA) Y. Yang in response to an Unusual Incident Report (UIR) received on 05/27/2026 regarding a child who sustained an injury requiring medical attention while in care. During the visit, the LPA met with Director Melissa Terhune to discuss the incident and obtain additional information. The director reported that, around afternoon pick-up time on 05/22/2026, child C1 was walking from the elevated, padded play structure area to the adjacent ground-level area outside the structure's boundaries, which was approximately eight inches lower in elevation. According to the director, C1 fell while transitioning between the two areas and landed on the wood-chip surface below. Staff member S1 was nearby, observed the incident, and offered C1 an ice pack, which the child declined. The director reported that C1’s authorized representative arrived at the facility approximately three minutes later to pick up the child. After C1 complained of discomfort, the authorized representative transported the child to a medical professional for evaluation. The director later learned from the authorized representative that C1 had sustained a fractured elbow as a result of the fall. During today's inspection, the LPA reviewed the facility’s child supervision policies with the director and inspected the outdoor play area. The LPA observed that the play structure and surrounding play area involved in the incident were age-appropriate for the children in care. The LPA also observed that the area beneath and surrounding the play structure was covered with artificial turf installed over soft foam cushioning material, and that the adjacent area contained sufficient wood-chip cushioning.The incident was reported to the Community Care Licensing Division (CCLD) and the child’s authorized representative as required by regulations. Additionally, the LPA provided technical assistance to the director regarding Title 22 regulations and staff and child ratios. Based on available information, it has been determined that there was no evidence of violations of regulations that occurred that lead to the child's injury. This report was read and reviewed with the center director, Melissa Terhune. There were no Title 22 violations cited during today's case management visit. The Notice of Site Visit shall be posted for 30 days.
