What the inspector wrote

On 03/20/24, Licensing Program Analysts (LPAs) Anita Tristan, Valerie Mireles and Licensing Program Manager (LPM) Cynthia Brannon arrived at the facility to conduct an unannounced Case Management- Incident Inspection. LPAs and LPM met with Director, Carol Thomas. LPAs and LPM toured the facility inside, outside, and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office, on 02/12/24. On 02/09/24, Child #1 sustained an injury to the right pointer finger. Child #1 received medical treatment and was diagnosed with a fracture to the finger and received two stitches. Child returned to facility the following Monday at normal scheduled time. LPA Tristan interviewed staff, reviewed files and facility records. During today’s inspection of the outdoor playground, LPA Mireles and LPM Brannon observed a brown wooden balance beam with chipped paint and the base of the wooden fence with a broken plank at the preschool play yard. Per director a repair order has been submitted for the balance beam. This unusual incident appears to be an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements. Exit interview conducted and report was reviewed with Director, Carol Thomas. Appeal rights were provided and discussed. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
