Inspector’s narrative

What the inspector wrote

On May 1, 2026, Licensing Program Analysts (LPAs) Miguel Herrera and Priscilla Zamudio conducted an unannounced case management inspection at the facility. LPAs met with Director, Ariel Babbitt who accompanied LPAs during a tour of the facility both inside and outside. LPAs explained the purpose for the inspection. During the facility tour LPAs observed that the facility did not provide proper accommodation for children that are being potty trained. While in the Two’s classroom LPAs observed a ratio of 12 children to 1 teacher. However, to ensure ratios and supervision staff #1 was observed changing child #1’s diaper outside the bathroom, while the child stood with his diaper down which violated the child’s personal rights. Furthermore, LPAs witnessed another incident pertaining to a violation of a child’s personal rights. LPAs witnessed staff #2 yelled at child #2 as a form of discipline. The incident occurred in the PreK/TK classroom during lunch time. Director Babbitt immediately intervened to mitigate the situation. Staff #2 acknowledged that she yelled at child #2 out of frustration and apologized for her conduct. Although the child was demonstrating challenging behaviors the form of discipline was inappropriate and violated child #2’s personal rights. Exit interview conducted and report was reviewed with Director, Ariel Babbitt. Appeal rights were provided. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is cited: (see LIC. 809-D). Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of Acknowledgement of Receipt of Licensing Reports (LIC 9224) was given to Director, Ariel Babbitt. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Furthermore, staff #2 yelled at child #2 as a discipline methond to control the child's behavior, which posed an immediate risk to the health, safety, or personal rights to children in care.

Supervision shall include visual observation. This requirement is not met as evidenced by:Based on LPAs observations, the facility did not adequately maintain contastant supervison of children as they were observed running without shoes, muddy clothing and children playing in puddles. Furthermore children were observed climbing furniture and hanging from the play structure due to inadequate supervision, which poses/posed a potential health, safety or personal rights risk to persons in care.

(d) The surface of the outdoor activity space shall be maintained: ....This requirement is not met as evidenced by: During LPAs' inspection of the outdoor activity space LPAs observed the concrete... and grass surfaces posed tripping hazards as the surfaces had potholes, cracks and unlevel areas, which poses/posed a potential health, safety or personal rights risk to persons in care.

BUILDINGS AND GROUNDS
101238(a) Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times.This requirement was not met as evidenced by: Interviews and LPA observations revealed that classroom floors, furniture, restrooms and outdoor play areas were not well maintained. In the “Twos” classroom, the floor areas around tables were stained and sticky. In the restroom, LPA observed dirt and grime on step stool and floor corners. Toilets were unflushed with urine stains on toilet seats. Toilet bases also showed exposed rusty bolts and grime. Toilet paper was scattered throughout the restroom. In the outdoors, children's socks were in the mud and several cleansing wipes were scattered on the ground.
