What the inspector wrote

On 08/03/2023, Licensing Program Analysts (LPAs) Valerie Mireles and Ka Vang met with Director Kim Hammond for an unannounced case management inspection. A complete file review was conducted prior to today's inspection. LPAs toured the facility. Currently, no children were present due to facility being temporarily closed for staff in-services. The purpose of today's inspection was to address an unusual incident that took place at the facility on 07/18/2023. An Unusual Incident Report was submitted to the Fresno Community Care Licensing (CCL) Office regarding an incident that occurred on 07/18/2023 involving a daycare child that fell while rolling over the body roller on the indoor play structure resulting in the child sustaining a fracture to the child's left elbow. On 08/03/2023, LPAs spoke with Staff #1, who reportedly witnessed the incident as it occurred and was in proximity of the child when the incident occurred. According to the staff, the child climbed the indoor play structure and the child rolled over the body roller and child landed on child’s left elbow. Initially, child cried and complained of pain to child’s rib cage area. Per Staff #1 and Staff #2, no visible bruises and/or opened wound; however, child was checked and observed. During picked up, child’s parent was notified. Per Director, child was taken by parent to the emergency room and saw a physician on the date of the incident and was diagnosed with a fracture to child’s left elbow and child was referred to and met with an orthopedic specialist on 07/28/2023. On 07/28/2023, the child returned to the facility with a doctor's clearance and arm brace. The Director reported that they will be able to meet and accommodate the child's needs. On 08/03/2023, LPAs observed the indoor play structure where the incident took place. The indoor play structure was inspected and was in good condition with no visible defects and the body roller rolled smoothly. At the time of the incident there were two teachers present and approximately eight children present in the indoor play structure room where the incident occurred; therefore, adequate supervision was in place. Based on the information obtained, LPAs determined staff handled the incident correctly and reporting requirements were met. After interviewing staff and reviewing facility records, LPAs determined facility staff took appropriate measures to address the child’s injury, following proper policies and procedures and no regulations were violated. Per Director, she stated that she will take further measures by having a staff inside the indoor play structure near the body roller to assist children as they roll over. Per the California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today’s inspection. Exit interview conducted with the Director, Kim Hammond.
