On 5/3/23 Licensing Program Analysts (LPAs) Joseph Pacheco and Araceli Gibson, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Director, Staci Coelho , and toured the facility indoors and outdoors. This facility operates on a traditional school year schedule. Days and hours of operation are Monday \342\200\223 Friday, 7:00am \342\200\223 5:00pm. Director verified facility phone number is (209) 667-5512. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. LPAs observed a broken sprinkler pipe in the outdoor activity area used by Classroom A that is causing a muddy puddle in the grass area and a broken sink in one of the bathrooms. LPA did observe toilets with exposed rusted bolts. Handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. LPA observed solid waste in a trash can that did not have a tight fitting lid. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Menus are posted at least one week in advance where an authorized representative can view them. Capacity and limitations as specified on the license are being maintained. LPAs did not observe any staff who have a current pediatric CPR card from an approved vendor. The name of the child care center (CCC) director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. CONTINUED ON LIC809-C Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening. LPAs observed multiple staff files that did not have documentation of required immunizations against measles, pertussis and influenza. All staff files reviewed did not contain documentation of a current AB 1207 Mandated Reporter certification. LPAs observed one staff who is not fingerprint cleared according to Guardian and Licensing Information Systems. Director was unable to provide evidence that Staff #1 has been fingerprint cleared as required. Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D). Director was provided a copy of their appeal rights. CONTINUED ON LIC809-C LPA Joseph Pacheco informed Director, Staci Coelho that this report dated 5/3/23 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Joseph Pacheco informed the Director to provide a copy of this licensing report dated 5/3/23 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Director, Staci Coelho. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
