What Happened

Annual inspection identified multiple facility maintenance issues including broken sprinkler pipe and sink, exposed rusted bolts on toilets. Significant staff record deficiencies noted: no current CPR certifications, incomplete immunization records, missing Mandated Reporter training, and one uncertified staff member. Type A citation issued for lack of CPR certification.

Based on record review. According to documentation provided by Director, a fire drill was last conducted on 8/31/22. This poses a potential health, safety or personal rights risk to persons in care.

Based on observation. LPA did not observe a water dispenser in any of the four classrooms. Director stated that staff have to take children's water bottles for children who attend Classrooms A, B & C to be filled out of the faucet in the kitchenette adjacent to Classroom C. Staff in Classroom D have to take children to the water fountain located in the hall way outside of the room to fill their water bottles. This poses a potential health, safety or personal rights risk to persons in care.

Based on record review. During today's inspection LPA Pacheco observed Staff #1 providing care and supervision to children. Staff #1 identified themselves as a teacher. LPA reviewed Guardian and Licensing Information Systems and did not see a fingerprint clearance on file for Staff #1. Director was unable to provide documentation that Staff #1 has been fingerprint cleared which poses an immediate health, safety or personal rights risk to persons in care.
